Prepare a description of your invention. There is no special form, so you can do this yourself.
However, be certain that the description is sufficiently clear so that one having ordinary skill in the field
of your invention can understand it. Also, the Disclosure Document must be limited to written matter or
drawings on paper or other thin, flexible material, such as linen or plastic drafting material, having
dimensions or being folded to dimensions not to exceed 8 by 13 inches (21.6 x 33 cm.). Photographs also
are acceptable. Each page should be numbered. Text and drawings should be sufficiently dark to permit
reproduction with commonly used office copying machines.
(1) The Mechanics:
Attach to the description a $10 check made out to “The Commissioner of Patents and Trademarks,”
and a cover letter, in duplicate, stating:
The undersigned requests that the enclosed papers be accepted under the Disclosure Document
Program and that they be preserved for a period of two years. A check for $10 to cover the
governmental fee is enclosed herewith.
Please stamp the enclosed copy of this letter with the date of receipt and the Disclosure
Document number and return it to me.
